How to Register

The doctors welcome new patients who live within our practice area.

Please ask for a registration form at reception or click on the link below and print off a registration form. You will need to bring this into the practice with photographic ID (For anyone over the age of 16) and a utility bill for proof of address.  In additional you will also need to bring along your NHS number for us to be able t find you on the system.

As it often takes sometime for records to be forwarded from your former practice all newly registered patients will be asked to complete a health questionnaire and asked if you are on any regular repeat medication.  If so you will need to see a GP before your next prescription is due.

Temporary residents

If you are staying locally within the normal practice area you can see a GP at the practice for urgent medical care. You will be required to come in to the practice with photographic ID and complete a registration form prior to a consultation with a GP. Only urgent appointments will be allocated for temporary residents and this will normally be following a telephone consultation with our Duty GP.

We see patients at the following location. You must, however, register at either our Leeds Road or Pannal Surgeries:
