Also known as Detailed Coded Data, this can include basic
As of the 1st April 2016 the Leeds Road Practice may offer these services upon the request of the patient. The request will be submitted for approval by the practice and you will be informed of the decision.
Age of Access for Online Services
The practice offers ‘no’ online services to patients aged between 11 and 16 years. NHS England recommends that practices adopt a very cautious approach to allowing parents’ access to their child’s medical record once the child reaches the age of 11, especially in light of the extended access to medical records in 2015.
Children vary in the age at which they are able to make an independent and informed decision about who should have access to their record. The Leeds Road Practice has decided to adopt the most practical approach and withdraw parents’ access as soon as the child reaches 11 unless there are exceptional circumstances.
The practice understands that in some extreme circumstances access to a child’s online record may be required. If you think this applies your child please contact the surgery in writing.
