How to Register
The doctors welcome new patients who live within our practice area.
Please ask for a registration form at reception or access the “Download Forms ” section below and print off the relevant registration forms.
All patients need to complete the GMS1 Registration form as well as the Adult or Child Forms. Please be aware that patients between the age of 11 – 16 are not eligible for online services.
Acceptance on to the list is as at the discretion of the Doctor and we will ask you for proof of identity. (Please refer to our registration policy below and why we ask for this).
As it often takes some time for records to be forwarded from your former practice all newly registered patients will be asked to complete a health questionnaire and asked if you are on any regular repeat medication. Please complete all parts of the form to help the GP with your care. If you have repeat medication, you will need to see the GP before your next prescription is due.
There are a number of reasons why you may not be able to register with your chosen GP, for example you may live too far away. If this is the case simply choose another GP in your local area.
